National CASA standards for local programs require CASA volunteers to have contact with the child(ren) they serve every 30 days. If you are unable to see your CASA child during this time frame, you must complete and submit this form to your Advocate Supervisor. Acceptable reasons for not seeing your CASA child include distance, illness, incarceration or the child being on runaway status. 

CASA volunteers must follow this template for all court reports. Please remember court reports must be submitted to your Advocate Supervisor 15 days prior to the court hearing.
